How To Fill JEE Main 2021 Self Declaration Form / Undertaking? – Explained

With the countdown nearing its end for the first session of JEE Main 2021, a question that many students are asking is – How to fill the JEE Main 2021 self declaration form? Or, How to fill the JEE Main 2021 undertaking?

This is a new addition to the list of documents that one has to take to the center. The undertaking or the self declaration is a part of the JEE Main 2021 admit card. Candidates get it when they download the admit card document. However unlike the hall ticket itself, which is to be simply taken to the center, the undertaking has some fields which you need to fill up. Students are a little baffled as to what to fill in the four points, with different fields. So here we are explaining to you how to fill it up correctly.

How To Fill JEE Main 2021 Undertaking / Self Declaration Form?

First, you have to download all 3 pages of the admit card from the JEE Main 2021 login. In the page 1, there is admit card with the details and then there is the Undertaking / Self Declaration Form. Page 2, and 3 are important instructions for candidates and advisory for candidates regarding COVID-19.

Now, print all the pages of the admit card including the Undertaking / Self Declaration Form. It looks like the one below:

Now comes the part where you have to fill it. Here is how to do it.

Point #1 in JEE Main 2021 Self Declaration Form – That, I have read the Instructions, Guidelines and relevant orders pertaining to COVID-19 pandemic. I have read Information Bulletin, Instructions and Notices related to this examination available on the website ‘https://jeemain.nta.nic.in’

You do not have to write or do anything in this point.

Point #2 (a) in JEE Main 2021 Self Declaration Form – I have in the last 14 days(please tick , wherever it is applicable to you, otherwise leave blank): a) The following flu-like symptoms:

This has the following fields, and this is how to fill it:

Fever: Put a tick here if you had fever in the last 14 days

Cough: Put a tick here if you had cough in the last 14 days

Breathlessness: Put a tick here if you had breathlessness in the last 14 days

Sore throat / Runny Nose: Put a tick here if you had sore throat or runny nose in the last 14 days

Body ache: Put a tick here if you had body ache in the last 14 days

Other- Please specify: Write here if you had any other illness symptoms in the last 2 weeks time.

Point #2 (b) in JEE Main 2021 Self Declaration Form – been in close contact with a confirmed case of the COVID-19.(‘Close contact’ means being at less than one meter for more than 15 minutes.

You have to tick in the box here if you were in close contact with any person who was a CONFIRMED COVID-19 case. Close contact does not mean that you were simply talking to that person or something. It means that you were within 1 meter (~3 foot) distance, for more than 15 minutes. So this means you visited such a person and sat or stood just beside them almost. Only then, tick this.

Point #2 (c) in JEE Main 2021 Self Declaration Form – not been in close contact with a person suffering from COVID-19 and am NOT under mandatory quarantine.

If you are yourself not in mandatory quarantine, and also you did not meet anyone with COVID, then tick this.

Point #2 (d) in JEE Main 2021 Self Declaration Form – travelled the following cities/ country in the last 14 days prior to arriving at the Centre.

In this section there is a table like the following

Did you go to any other city or country in the last two weeks? If not, then you do not have to fill anything in this field.

If you did, then the first thing you have to fill is the name of that city or country in 1st city, against name field. Likewise fill all the names of places you have been to (in other city or country) in the last 2 weeks. In the second row, you have to fill the date on which you came to the city where your JEE Main 2021 exam centre is.

Point #3 in JEE Main 2021 Self Declaration Form – The health and wellbeing of our community is our first priority; therefore the centre reserves the right to deny entry to its premises.

Again, here you do not have to fill anything.

Point #4 in JEE Main 2021 Self Declaration Form – I have read the detailed “IMPORTANT INSTRUCTIONS for CANDIDATES” as given on Page-2 and “ADVISORY for CANDIDATES REGARDING COVID-19” as given on Page-3 and I undertake to abide by the same.

Here also you do not have to fill anything. But read the advisory and instructions section of your admit card properly.

Please paste here the passport size photograph which you uploaded in your JEE Main 2021 application form. You have to paste this BEFORE REACHING THE CENTER.

Please put your left hand thumb impression here. Use a blue ink pad. Press your thumb on the ink pad and then press your thumb in this space. Make sure the impression is clear. Do not press too lightly or too hard as then the impression will either be too light or get smudged. Apply enough pressure to get a good impression. You can first try on a few white papers and then do this on admit card. You have to do this BEFORE REACHING THE CENTER.

Things To Take Care of While Filling The Undertaking / Self Declaration Form

You must take care of the following important things while filling the JEE Main 2021 Undertaking / Self Declaration Form:

A) Please take care that you are filling in all details correctly. If you make a mistake, do not over-write or use any correcting ink. Take a fresh print of the undertaking and fill it again.

B) Use black / blue dot pens to fill the form. Avoid using coloured pens, markers, ink pens, etc. They can become messy and smudge.

C) Fill the undertaking completely. Leave the checkboxes / spaces if it is not applicable to you. Fill in all details except for the signature. You to do the sign in presence of the invigilator on the exam day.

D) Fill the undertaking authentically. This document is very important. Do not provide incorrect or misleading information here.

What is the Importance of the JEE Main 2021 Undertaking / Self Declaration Form

A self declaration form is a statement from the candidates that they are responsible for their health condition on the day of the exam. The details to be filled in the form will act as candidates statement and if any of these statement proves to be false, the candidate will be responsible for the consequences and not NTA or exam centre officials.

Thus, it is important to fill the self declaration form correctly with accurate information.

About JEE Main 2021

JEE Main stands for Joint Entrance Examination Main. It is one of the biggest engineering entrance examinations in India. NTA conducts it for admission of candidates in NITs, IIITs, CFTIs, etc., in B.Tech / B.E / B.Arch / B.Plan courses. Only eligible 10+2 qualified / appearing candidates can apply for the exam.
